We will require proof of name or address changes so please bring this with you on your next visit to the practice.
Please also remember to inform any hospital that you may be receiving treatment from. This is important in order that the information held is up to date and that all correspondence is sent to the correct address.

Please note, if you are moving out of the practice area you may be asked to re-register at a practice nearer to your new address. The practice can provide further guidance regarding this. As of 2021 we do not accept new registrations for patients outside of the practice boundary or patients moving outside of the practice contractual boundary.
