Registering at the practice

Patients wishing to register at the practice should complete a registration form obtainable from the main reception desk. You will need to provide a form of photographic ID and a utility bill to prove your residence. Once completed and returned your details will be entered on our system and the process of requesting your notes from your previous practice will commence.   It may take a few weeks before your notes arrive at the practice and so you will be offered a new patient check with the practice nurse, who will go over any previous medical history and cover any basic health checks, such as blood pressure and weight.

Before you attend for the new patient check, it would be very helpful if you completed the online medical questionnaire (below)  so that this information will be readily availble prior to your appointment with the nurse.  Please note this form is NOT a registration form for joining the practice.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
